Gloribi Pastor, RN, has been a valued member of Elmwood Adult Day Healthcare since joining the organization in September of 2017.
As a Registered Nurse, Gloribi plays a pivotal role in managing the clinical and operational aspects of the program, with a particular focus on nursing care, medication administration, and treatment management. Her daily responsibilities include overseeing the development and review of individualized care plans, ensuring that each participant receives the appropriate level of care and treatment based on their needs. This includes conducting initial assessments, formulating 90-day care plans, and regularly reevaluating and adjusting care strategies in collaboration with the interdisciplinary care team.
In addition to her clinical duties, Gloribi is responsible for managing the day-to-day operations of the facility. She ensures that all staff members adhere to the required standards set forth by regulatory agencies, including the Department of Health (DOH), the food program, OSHA, and union guidelines. Gloribi is also heavily involved in staff management, including hiring, conducting interviews, and ensuring that all personnel are following Elmwood Adult Day Healthcare's standards and employee handbook.
A key aspect of her role involves fostering open communication and collaboration within the team. Every Tuesday, Gloribi holds a staff meeting at the 209 Elmwood Avenue location, where she facilitates discussions on ways to improve the program, addresses challenges, and develops interventions to enhance overall care delivery.
Gloribi’s daily routine is both dynamic and hands-on. Upon arriving, she greets staff and participants and performs a quick assessment of the facility to ensure medication administration is proceeding according to schedule. She carefully monitors food logs and staff attendance to maintain compliance with established protocols. Afterward, she checks the temperature of the refrigerator to ensure food safety standards are met and administers medications to participants, closely monitoring vital signs such as blood pressure and glucose levels. When necessary, she communicates any significant changes to the participants' primary care physicians and adjusts the care plan accordingly.
In addition to managing clinical duties, Gloribi addresses any escalation matters that arise throughout the day, including those related to transportation, participant care, and phone calls. She ensures that all aspects of the facility’s operations run smoothly, from reviewing Sysco food delivery invoices to coordinating specialty doctor appointments, arranging CNA home care services, and facilitating physical therapy for participants.
Currently, Gloribi is participating in the Food Manager Program Education to further enhance her knowledge and ensure that safety procedures related to food handling and preparation are strictly adhered to. She actively works with the team to implement these procedures, taking corrective action where necessary.
Beyond her clinical and managerial responsibilities, Gloribi also dedicates time to administrative tasks, such as updating care plans, nurse’s notes, and preparing for meetings. Her dedication to continuous improvement and her hands-on approach make her an indispensable member of the Elmwood Adult Day Healthcare team, as she works tirelessly to provide exceptional care and support to all participants.
