About the Role

As an Occupational Therapist (OT/R) at Elmwood Healthcare - Home Health (Elmwood Home Care), you will be responsible for assessing, planning, and implementing individualized therapeutic interventions to help clients achieve their highest level of independence and improve their overall quality of life. You will work closely with clients, families, and other healthcare professionals to deliver comprehensive and effective care in a home setting.

Key Responsibilities:

  • Assessment and Evaluation: Conduct thorough assessments of clients' physical, cognitive, and emotional abilities to determine their needs and develop appropriate treatment plans.

  • Treatment Planning: Develop and implement individualized therapy plans based on assessment findings, client goals, and evidence-based practices. Adjust plans as needed based on client progress and feedback.

  • Therapeutic Interventions: Provide direct therapy services to clients, including activities of daily living (ADL) training, fine and gross motor skill development, cognitive and perceptual training, and adaptive equipment recommendations.

  • Client and Family Education: Educate clients and their families about the therapeutic process, home modifications, adaptive techniques, and strategies to support clients' independence and safety at home.

  • Documentation: Maintain accurate and timely documentation of therapy services, client progress, and outcomes in compliance with company policies and regulatory requirements.

  • Collaboration: Work collaboratively with other healthcare professionals, including physicians, nurses, and social workers, to ensure coordinated and holistic care for clients.

  • Case Management: Monitor and evaluate client progress, adjust treatment plans as needed, and communicate regularly with the interdisciplinary team and clients' families.

  • Compliance and Safety: Adhere to all relevant regulations, standards, and best practices in home healthcare, ensuring a safe and effective therapeutic environment for clients.

  • Medication Management, Education and Reconciliation

  • Development and Teaching of Individuated Home Exercise Programming (HEP)

  • Strategic Therapies to Address Movement, Function and Strengthening

  • Gait, Mobility and Ambulation Training

  • Physical Reconditioning


  • ADL/IADL Training

Qualifications

  1. Education: Master’s or Doctoral degree in Occupational Therapy from an accredited program.

  2. Licensure: Current and valid Occupational Therapist license (OT/R) in the Commonwealth of Massachusetts.

  3. Experience: Minimum of one (1) year of experience as an Occupational Therapist, preferably in a home healthcare setting.

  4. Skills: Strong clinical assessment and therapeutic skills, excellent communication and interpersonal skills, ability to work independently and manage time effectively.

  5. Certifications: CPR Certification with Basic Life Support (BLS) or Advanced Life Support (ALS) is required.

  6. Other Requirements: Valid driver’s license, reliable transportation, and willingness to travel to patients' homes within the designated service area.
