About the Role
The Home Health Aide (HHA/CNA) at Elmwood Healthcare - Home Health (Elmwood Home Care) will provide essential personal care and support services to patients in their homes, assisting them with daily living activities and ensuring their comfort and well-being. You will work under the supervision of a registered nurse or healthcare professional to deliver care that meets the individual needs of each patient.

Key Responsibilities:
Personal Care:Â Assist clients with activities of daily living (ADLs) such as bathing, dressing, grooming, toileting, and eating. Provide support with mobility, transfers, and positioning.
Companionship and Emotional Support:Â Offer companionship and emotional support to clients, helping to alleviate feelings of loneliness and promoting a positive, supportive environment.
Medication Assistance:Â Remind clients to take prescribed medications as directed. Monitor for any changes in health and report concerns to the supervising nurse or healthcare professional.
Household Assistance:Â Perform light housekeeping tasks such as cleaning, meal preparation, and laundry to maintain a safe and comfortable living environment for clients.
Health Monitoring:Â Observe and report any changes in clients' physical or mental health to the supervising nurse or healthcare professional. Assist with measuring and recording vital signs if required.
Documentation:Â Maintain accurate and up-to-date records of the care provided, including any changes in clients' conditions, and ensure proper documentation is completed in accordance with company policies.
Safety and Compliance:Â Adhere to all safety and infection control protocols to ensure a safe environment for both clients and staff. Follow all relevant regulations and best practices in home healthcare.
Qualifications
Certification:Â Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification in the State of Rhode Island.
Experience:Â Previous experience as a Home Health Aide, Certified Nursing Assistant, or in a similar caregiving role is preferred.
Skills:Â Strong interpersonal skills, ability to provide compassionate care, excellent communication abilities, and capability to perform physical tasks required for caregiving.
Training:Â Completion of a state-approved training program for HHAs or CNAs. Current CPR and First Aid certification is required.
Other Requirements: Valid driver’s license, reliable transportation, and willingness to travel to clients’ homes within the designated service area.
