About the Role

The Homemaker is a vital member of Elmwood Healthcare’s Home Care (Non-Skilled) Division, providing essential non-clinical support services that enable clients to remain safely and comfortably in their own homes. This position focuses on maintaining a clean, organized, and functional home environment while promoting independence and overall well-being for clients who may be elderly, disabled, or recovering from illness or injury.


Working under the direction of a supervisor and in accordance with an individualized care plan, the Homemaker performs household tasks, meal preparation, and companionship services. The ideal candidate is dependable, respectful, and service-oriented, with a genuine commitment to supporting clients' dignity and quality of life.

Key Responsibilities:

  • Patient Care and Support:

    • Assist clients with activities of daily living (ADLs), including bathing, grooming, dressing, toileting, oral hygiene, and mobility assistance.

    • Provide support with ambulation, transfers (bed, chair, toilet), and use of assistive devices as needed.

    • Support safe meal preparation and feeding assistance in accordance with dietary restrictions or preferences.

  • Household Management

    • Perform routine light housekeeping tasks such as dusting, sweeping, vacuuming, mopping, laundry, and dishwashing.

    • Clean kitchens, bathrooms, bedrooms, and other living spaces to maintain a sanitary and safe environment.

    • Organize household items and ensure walkways and common areas are free of hazards to reduce the risk of falls or accidents.

  • Meal Preparation and Nutritional Support

    • Plan and prepare simple, nutritious meals and snacks based on client preferences and dietary guidelines outlined in the care plan.

    • Assist with meal setup and clean-up as needed.

    • Encourage client hydration and appropriate food intake.

  • Assistance with Activities of Daily Living (ADL's)

    • Assist with local errands such as grocery shopping, picking up prescriptions, or mailing items, if authorized and approved.

    • Support clients with making appointments or reminders for daily tasks, without providing direct personal care or medical services.

  • Companionship and Emotional Support

    • Provide friendly companionship and engage in light conversation or leisure activities to promote social interaction and reduce isolation.

    • Observe and report any significant changes in the client’s mood, environment, or behavior to the supervisor.

  • Health, Safety, Confidentiality and Compliance:

    • Adhere to infection control practices, safety protocols, and all relevant regulations to ensure a safe care environment.

    • Use medical equipment and supplies safely and appropriately.

    • Maintain strict confidentiality of all participant and organizational information.

    • Adhere to HIPAA and all regulatory guidelines related to healthcare documentation and privacy.

    • Complies and upholds the policies and procedures of Elmwood Healthcare and it's local, state and federal regulatory authorities.

  • Professional Conduct:

    • Demonstrate respect, professionalism, and cultural competence in all client interactions.

    • Attend required trainings, in-services, and agency meetings to maintain certification and enhance job knowledge.

    • Maintain valid certification and required health clearances as per state and agency requirements.

  • Ongoing Support and Professional Development

    • Participate in ongoing training and professional development activities.


Qualifications

  1. Education: High-School Diploma and Certified Nursing Assistance Certificate required.

  2. Licensure: Current, valid and unencumbered license to practice as a Certified Nursing Assistant (CNA) in the State of Rhode Island and Providence Plantations. No active, open or previously adjudicated disciplinary findings reported by the Rhode Island Department of Health, Board of Registration in Nursing or any other professional licensing Board therein.

  3. Experience: Minimum of two (2) years of nursing experience in elder care, community health or home health / home care settings.

  4. Skills: Ability to cultivate and maintain an engaging and stimulating environment in group settings; Excellent communication and interpersonal skills; Willingness to work through communication and health literacy barriers; Ability to work independently and manage time effectively.

  5. Communication: English proficiency is required for all candidates; Spanish proficiency (written, oral or both) is preferred.

  6. Certifications: CPR Certification with Basic Life Support (BLS) or Advanced Life Support (ALS) is required.

  7. Other Requirements:

    1. Valid driver’s license and reliable transportation.

    2. Written work clearance from an authorized medical provider in the State of Rhode Island and Providence Plantations.

    3. Ability to satisfy all pre-employment requirements as determined by those applicable policies of Elmwood Healthcare, including but not limited to: Vaccination and immunization records, freedom from communicable disease(s), successful completion of criminal background check.
