About the Role

The Director of Clinical Services is a Registered Nurse and key clinical leader responsible for advancing Elmwood Healthcare’s mission to deliver next-generation, community-based healthcare that is value-driven, quality-focused, equitable, and deeply responsive to the individualized needs of patients and families. In alignment with the organization’s vision to transform healthcare through continuity of care, collaborative relationships, clinical excellence, and innovative service delivery, the Director of Clinical Services provides leadership, supervision, and strategic oversight for skilled home health services furnished to patients in their place of residence.

Working collaboratively with the leadership team, the Director of Clinical Services ensures that all clinical services are delivered in accordance with physician orders, individualized plans of care, professional standards of practice, and the highest expectations of regulatory, accreditation, and organizational compliance. This role is integral to ensuring that Agency staff perform in alignment with the standards established by Elmwood Healthcare, the Centers for Medicare & Medicaid Services, The Joint Commission, and all applicable local, state, and federal regulatory authorities.

As the designated clinical leader for skilled services, the Director of Clinical Services oversees the supervision, direction, and development of clinical personnel while promoting a culture rooted in quality, integrity, compassion, humanity, equity, compliance, innovation, collaboration, and community. This position supports the organization’s commitment to improving patient outcomes, reducing avoidable re-hospitalizations, promoting independence, strengthening transitional safety, and ensuring that each patient receives care that is individualized, respectful, clinically appropriate, and outcome-oriented.

The Director of Clinical Services participates in all activities relevant to the organization’s licensure, accreditation, clinical operations, and professional services, including the development of staff qualifications and assignments, ongoing professional and skill-based education, performance improvement programming, survey preparedness, regulatory compliance initiatives, and clinical leadership planning. The Director remains available during operating hours, or designates a similarly qualified alternate when appropriate, to ensure continuity of clinical leadership, operational accountability, and excellence in care delivery across the Agency.

Key Responsibilities:

• Clinical Leadership: Ability, knowledge and confidence to effectively lead clinical teams and uphold quality standards across the Organization.

• Strategy and Quality: Provide strategic leadership for all clinical programs, ensuring alignment with organizational goals and regulatory standards.

• Clinical Support: Supervise and support clinical staff across disciplines including skilled nursing, physical therapy, occupational therapy, speech and language pathology and medical social work.

• Care Planning and Oversight: Oversee patient assessments, diagnostic evaluations, and treatment planning utilizing evidence-based practices.

• Documentation Accuracy and Quality Assurance: Ensure accurate medical documentation, compliance with HIPAA regulations, CMS Regulations (OASIS), Joint Commission Standards and proper management of medical records using ICD-10/ICD-9 coding systems.

• Performance Improvement: Develop and implement Performance Improvement initiatives.

• Education: Facilitate training programs on topics such as motivational interviewing, crisis management, behavior management, and working with children or individuals with disabilities.adherence to care plans.

• Patient Care: Provide direct nursing care, including administering medications, wound care, IV therapy, and other treatments as prescribed by the physician. Monitor and assess patients' conditions and adjust care plans as needed.

• Assessment and Evaluation: Conduct comprehensive assessments of patients' health status, including vital signs, medical history, and physical examinations. Develop and implement individualized care plans based on assessment findings and patient goals.

• Care Coordination: Collaborate with physicians, therapists, and other healthcare professionals to coordinate care and ensure a cohesive approach to patient management. Communicate effectively with the interdisciplinary team to address any changes in patients' conditions or needs.

• Patient and Family Education: Educate patients and their families on disease management, medication administration, self-care techniques, and lifestyle modifications. Provide guidance and support to help patients and families manage health conditions and enhance their quality of life.

• Documentation: Maintain accurate, detailed, and timely documentation of patient care, including progress notes, treatment plans, and any changes in patients' conditions. Ensure documentation complies with company policies, insurance requirements, and regulatory standards.

• Health and Safety Compliance: Adhere to infection control practices, safety protocols, and all relevant regulations to ensure a safe care environment. Use medical equipment and supplies safely and appropriately.

• Emotional Support: Provide compassionate support to patients and their families, addressing concerns and offering comfort and reassurance as needed. Foster a positive and supportive relationship with patients to encourage
