About the Role
Elmwood Adult Day Health Care Center is seeking a compassionate and skilled Registered Nurse (RN) to provide direct nursing care to our participants, with a special focus on supporting individuals affected by cognitive impairments. The RN will play a vital role in promoting the health and well-being of participants by administering medications, conducting assessments, and supporting the development of personalized care plans. Working under the supervision of the Nursing Director, the RN will collaborate with an interdisciplinary healthcare team to deliver holistic, person-centered care in a safe, respectful, and nurturing environment.

Key Responsibilities:
Patient Care:Â
Administer medications and treatments in accordance with individual care plans.
Conduct nursing assessments to monitor and evaluate participant health.
Ensure prompt and effective care delivery tailored to participant needs.
Maintain accurate and up-to-date medical records for all participants.
Assessment and Evaluation:
Conduct comprehensive assessments of patients' health status, including vital signs, medical history, and physical examinations.
Develop and implement individualized care plans based on assessment findings and patient goals.
Care Coordination and Planning:
Assist in developing and updating individualized care plans.
Conduct weekly care planning meetings in collaboration with the Nursing Director.
Work with interdisciplinary team members to address changes in participant conditions.
Collaborate with physicians, therapists, and other healthcare professionals to coordinate care and ensure a cohesive approach to patient management. Communicate effectively with the interdisciplinary team to address any changes in patients' conditions or needs.
Patient, Family and Staff Education and Communications:
Educate patients and their families on disease management, medication administration, self-care techniques, and lifestyle modifications.
Provide guidance and support to help patients and families manage health conditions and enhance their quality of life by providing health literacy.
Serve as a point of contact for staff and families regarding participant health and care.
Foster open, respectful communication with participants, caregivers, and colleagues.
Support staff in resolving participant care issues.
Documentation:
Maintain accurate, detailed, and timely documentation of patient care, including progress notes, treatment plans, and any changes in patients' conditions.
Ensure documentation complies with company policies, insurance requirements, and regulatory standards.
Health, Safety, Confidentiality and Compliance:
Adhere to infection control practices, safety protocols, and all relevant regulations to ensure a safe care environment.
Use medical equipment and supplies safely and appropriately.
Maintain strict confidentiality of all participant and organizational information.
Adhere to HIPAA and all regulatory guidelines related to healthcare documentation and privacy.
Complies and upholds the policies and procedures of Elmwood Healthcare and it's local, state and federal regulatory authorities.
Emotional Support:
Provide compassionate support to patients and their families, addressing concerns and offering comfort and reassurance as needed.
Foster a positive and supportive relationship with patients to encourage adherence to care plans.
Ongoing Support and Professional Development
Assist the Nursing Director and healthcare team in special projects or clinical tasks.
Participate in ongoing training and professional development activities.

Qualifications
Education: Associate's, Bachelor's Master’s or Doctoral degree in Nursing from an accredited program.
Licensure:Â Current, valid and unencumbered license to practice Registered Nursing (RN) in the State of Rhode Island and Providence Plantations. No active, open or previously adjudicated disciplinary findings reported by the Rhode Island Department of Health, Board of Registration in Nursing or any other professional licensing Board therein.
Experience:Â Minimum of two (2) years of nursing experience in elder care, community health or long-term care settings.
Skills:Â Strong clinical assessment and therapeutic skills; Excellent communication and interpersonal skills; Willingness to work through communication and health literacy barriers; Ability to work independently and manage time effectively.
Communication: English proficiency is required for all candidates; Spanish proficiency (written, oral or both) is preferred.
Certifications:Â CPR Certification with Basic Life Support (BLS) or Advanced Life Support (ALS) is required.
Other Requirements:
Valid driver’s license and reliable transportation.
Written work clearance from an authorized medical provider in the State of Rhode Island and Providence Plantations.
Ability to satisfy all pre-employment requirements as determined by those applicable policies of Elmwood Healthcare, including but not limited to: Vaccination and immunization records, freedom from communicable disease(s), successful completion of criminal background check.
